Burn permit application

Please print and complete the application legibly, completely, sign, initial, and email to: 

westhennepin@westhennepin.com 

The application will be reviewed by staff then emailed back to you once approved.

Permits are issued in one month increments and you can request multiple months.

Permit Fees are $10.00 per month

All Permits expire on Dec 31 of each year.

• Permittee Information
  • Full Name of Applicant
  • If owner is different than applicant enter Representing Name. 
  • Address and Phone for Applicant
• Burnsite Location
  • Physical Address of Burn
• Burning Permit Conditions
  • Check box for type of burn 
  • Completely fill out checked section
  • For Piled Material, circle fuel type, and you must enter # of Piles and Pile Size. example - # Piles 1  Approx. Pile Size 10'x10'
• Sign and Date
  • Page 1 sign & date
  • Page 2 initial
• Mail or Drop off permit fee to WHPS
  • $10.00 per month of permit requested. (example: 3 month permit = $30.00)
  • Make Checks payable to West Hennepin PSD.
